APOE encodes apolipoprotein E, a lipid-transport protein that packages cholesterol into lipoprotein particles for transport in blood and, in the brain, mediates lipid delivery to neurons and clearance of amyloid-beta. The combination of rs429358 and rs7412 defines the e2/e3/e4 alleles, and the e4 allele is the strongest common genetic risk factor for late-onset Alzheimer's disease, largely through reduced amyloid clearance.
