CYP2D6

CYP2D6 encodes a highly polymorphic hepatic cytochrome P450 enzyme that metabolizes an estimated 20-25% of commonly prescribed drugs, including many antidepressants and antipsychotics, opioid analgesics such as codeine and tramadol, and beta blockers. Its activity is shaped not only by single-nucleotide variants but also by gene copy-number changes and hybrid/deletion alleles, so accurate phenotype prediction (poor, intermediate, normal, or ultrarapid metabolizer) generally requires whole-genome-level structural variant calling rather than a handful of SNPs.
