UGT1A1
UGT1A1 encodes UDP-glucuronosyltransferase 1A1, a liver enzyme that conjugates glucuronic acid onto bilirubin (and onto SN-38, the active metabolite of the chemotherapy drug irinotecan) to make them water-soluble for excretion. Reduced-function alleles such as UGT1A1*28 lower glucuronidation capacity, causing mildly elevated unconjugated bilirubin (as in Gilbert syndrome) and increasing the risk of severe toxicity from irinotecan and other UGT1A1 substrates.
